Nursery and Reception Applications

If you would like to apply for your child to join our nursery or reception please follow this link to the Stoke-on-Trent City Council Website and follow the steps to complete the application. 

Alternatively, complete the below form and return it to the office. 

Medicine

If your child is prescribed antibiotics 4 times a day we are able to administer them during the school day. However, we would need you to complete an administration of medicine form. You can find a copy of the form below or pop into the office to complete it. 

Free School Meals

30hrs Funding
